Applying for your CAIPS file is easy.

5. Pay the service fee
6. Download, complete, sign and return the IMM5475 Form(s) to us

4. Complete and submit the Online Registration Form
Complete our simple Online Registration Form and click Submit. By submitting this form, you agree with our Terms of Service.
5. Pay the service fee
Pay securely online through PayPal, the secure online payments world leader. Besides your PayPal account, you may use VISA, MasterCard, AMEX, DISCOVER or eCHECK. Please make sure PayPal is available in your country. Select the service as per the Online Registration Form and click Buy Now. Please be advised that we are unable to provide the service until the online payment is cleared.

6. Download, complete, sign and return the IMM5475 Form(s) to us
IMM5475, "Authority to Release Personal Information to a Designated Individual" is an official CIC form on which you provide your consent for CIC to disclose the information in your CAIPS file to us.
Your spouse or common-law partner can sign the same form. Your dependent children who are 18 years of age or older have to provide separate completed and signed IMM5475 forms.

Please do not resize the form when printing or scanning.
The IMM5475 Form has just 11 boxes to be completed:
- After "Family name (Surname)", put your family name;
- After "Given name(s)", put your given name(s);
- After "Your date of birth", put your date of birth in the boxes provided;
- After "Name of the office where the application was submitted", put name of the visa office which is handling your visa application e.g. "CHC";
- After "Location of office", put the location of the visa office which is handling your visa application e.g. "London";
- After "Type of application (permanent residence, extension of study permit, etc.)", put the type of application e.g. "permanent residence";
- After "Your Client Identification (ID) or Unique Client Identifier (UCI) number identification number (if known)", put your file number e.g. "B123456789". All forms should have the same client number;
- After "Signature of applicant", sign in the box provided;
- After "Date", put the current date in the boxes provided;
- After "Signature of spouse or common-law partner (if applicable)", have your spouse or common-law partner sign in the box provided (if applicable);
- After "Date", put the current date in the boxes provided.
Return the completed and signed form(s) to us

7. Send us your CAIPS file
